APPLICATION FOR PERMISSION TO RECEIVE
PASSPORTS OF MINORS (1) ABC, DATE OF BIRTH:
Respectfully Sheweth:

That the applicant has filed an
application for appointment of guardian for the person of the minors (1) ABC, DATE OF BIRTH:

That on the above said
application, the applicant was appointed person of the said minors vide Guardianship
Certificate dated

That the applicant wants to
obtain/ receive passports of above said minors from the Passport Office, and
want to take the minor abroad.
PRAYER:
Under the foregoing
circumstances, it is respectfully prayed that instant application may kindly be
accepted and applicant be allowed to
obtain /get passport of minors and to take the minors abroad.
Any other relief which
this Honourable Court may deem fit and
proper, may also be awarded.
