I, Joseph Masih Guddu son of Jonson Nazima Guddu Date of Birth: 01-03-1987 CNIC/NICOP No. 374544-77855544-3 do hereby solemnly affirm and declare on oath that I have left my inherited religion and converted to religion of Islam.

 

My previous name is Joseph Masih Guddu S/O Jonson Nazima Guddu and I have changed my name, my new Islamic name is Salamat Hussain.  

 

I hereby assure NADRA authorities that the above statement is based on fact and will be liable for any legal action, if any subversive intentions are detected on my part later on.
