AFFIDAVIT
I, Raja Malik Gujar Nabeel SA/2500000 son of Gujar Raja Khan resident
of House No. 1, Street No. 001, Mohallah No. 002, Gawalmandi, Rawalpindi,
employee of PMAD hereby undertake the responsibility to repay the HP Price and
surcharge levied in case of non/ delayed payment of installments against Suzuki
Altu VXL (item) purchased under CSD Hire Purchase Scheme within stipulated
period of payment schedule. In case some CSD dues are outstanding at the time
of retirement/ premature retirement/dismissal, will also be paid by me before
retirement and if I became deserter or tried by

undertaking statement to the same effect) I also hereby undertake that I shall abide
by the terms and conditions hereinafter contained in relevant Standing
Operating Procedure (SOPs) of hire
purchase scheme issued by CSD from time to time, failing which Canteen Store
Department (CSD) has legal right to refer the case before court of law or respective Pension Department through
appropriate channels for recovery of outstanding amount against my names. That
what is stated above is true and correct to the best of my knowledge and belief
nothing has been kept concealed.
